You and your dependents become eligible for health and life insurance benefits on the first day of the month following 30 days of employment.  All full-time employees and employees working half-time or greater are eligible for coverage.  Once you select your benefit, you will have the opportunity to make a change during the Open Enrollment period, which occurs once a year, in late October.  The elections you make during this enrollment will be in effect until the next open enrollment occurs.  As an eligible employee, you can also enroll your eligible dependents.

Your dependents include:

your legally married spouse;

bullet

unmarried dependent children up to the end of the calendar year in which they reach age 19, provided they depend upon you for support;

bullet

unmarried children up to age 25 who are full-time or part-time students and who depend upon you for support.  Verification of school enrollment is necessary.

Open Enrollment:  Each year, during Open Enrollment, you will have an opportunity to make new benefit elections for the coming year.  These changes go into effect January 1 of the following year.  All eligible employees who wish to make any benefit changes must complete an enrollment/change form.

Flexible Benefit Plan:  In addition to your Core Benefits, you have the opportunity to select benefits from the Flexible Plan and have premiums deducted from your check on a pre-tax basis through our Section 125 Plan.  The IRS has established rules, which state that once you have enrolled for the plan year, you cannot change until the next annual enrollment, unless a qualifying event occurs.

The Benefits Office must be contacted within 30 days of the qualifying event, in order to make a change.  If you do not contact the Benefits office within the 30 days, then you are required to wait until the next annual enrollment.  A qualifying event includes any of the following changes:

Marriage or Divorce

Birth or Adoption of a Child

bullet

Death of a Spouse or Child

bullet

Change in Spouse's employment or health insurance that affects your family benefits

bullet

Change in residence of employee, spouse or dependent

**Please note that as a 10-month employee if you work through your last contracted day (June), your insurance is good until September 30 and you will automatically be sent a COBRA notice.**
