IMPORTANT INFORMATION

H1N1 (swine flu) mist will be available at school for FREE!

Good news! The Alachua County Health Department will be offering free H1N1 (swine flu) immunizations to all Pre-K through 12th-grade students in the county. Like the seasonal flu vaccine, the H1N1 vaccine will be given as a nasal spray (NO SHOTS!) at our school and will be free of charge. But parents must fill out and return the required consent form for their child to be eligible, just like they did for the seasonal flu immunization program. The consent form, an information sheet and an immunization calendar for the H1N1 program have been sent home with your child. They are also available on the district’s website at http://www.sbac.edu

Please return your consent form to school as soon as possible. If you send it back with your child, remind your child to give the form to his or her teacher. We must have a completed, signed consent form for your child before our school’s immunization date of December 3rd and 4th. If you have any questions, please contact the Alachua County Health Department at 334-7950.
