• Student Information/Emergency Card (Available in the Front Office)
• Birth Certificate
• Social Security Card
• Immunization and School Physical
• Two Proofs of Residence
  • Utility Bill
  • Cable Bill
  • Lease Agreement
  • Mortgage Statement
