Dear Chaperone,

The 7^th grade teachers at Kanapaha Middle School would like to thank you for your participation in our wonderful field trip on May 16^th.  To show our appreciation, the total cost of the trip for you will be $30.

Please read over the responsibilities and information before signing the bottom of the form.  It is important that you understand and agree to the following:

1.        Each chaperone will be responsible for up to 10 students.  These students will remain with the chaperone at all times.

2.       It is important that students belong to a group that has similar interests – same rides, same activities, etc.  Therefore, a chaperone sign up list will be created for students to register with the chaperone and group of students of their choice.

3.       All student permission slips will be left at the First Aid station at Epcot.  In the event of an emergency, please report to the First Aid station immediately and call one of the teacher chaperones.

4.       Chaperones are responsible for their own food.  Epcot does not allow coolers; therefore, plan to purchase food at the park.

5.       Please be at the school by 7:30 AM on May 16^th.  You will be given your roster and any further information at this time.

6.       If you have a cell phone, please bring it and register your number with Mrs. Bairley.  It will help tremendously with communication during the field trip.

7.       Chaperone cost for the trip, which includes bus transportation and a ticket to Epcot, is $30.  This must be paid in full by April 1^st.

8.       ONLY 7^th grade students from Kanapaha Middle School will be permitted to attend the trip.

9.       All chaperones are asked to attend an informative meeting a few days before the trip.  You will be contacted by email or phone when the exact date and time have been determined.
