- a. Have patron fill out a complaint form. (Citizens
Request Form for Re-Evaluation of Materials )
- b. When completed form is returned set up a committee
which includes a member of administration, the media specialist/s and
several language arts or reading teachers.
- c. Have committee members read the book in its entirety
and all the documents below.

- e. Recommendation is made to the principal to keep the
book on the shelves or pull it from the collection of that school. The book
is not pulled until the recommendation is made to the principal to do so.
