Middle and High School COVID Vaccine Clinics
The district is once again partnering with the Alachua County Health Department to provide FREE COVID vaccinations in school to eligible students (ages 12 and up). In August, clinics were held in all middle and high schools to provide students with their first Pfizer vaccination. Additional clinics have been scheduled to give those students their second dose or other students their first vaccination.
If you would like your child to receive the free vaccination, please submit the COVID Vaccine Consent Form (available here in Spanish) to your child’s school. Forms should not be submitted earlier than ten days before the clinic day at your school. Forms can be returned the day of the clinic, although we do ask parents to submit the forms ahead of time if possible.
IF YOUR CHILD WILL BE RECEIVING THE SECOND DOSE, please have them bring their vaccination card so the 2nd dose can be recorded. IF YOUR CHILD WILL BE RECEIVING THEIR FIRST VACCINATION, you will need to arrange for them to have their second dose in the community about three weeks later.

Vaccine Consent Forms
Students ages 12 to 17 must have a consent form signed by a parent or guardian for each vaccination. Students 18 and older may sign their own forms.
The consent form should be submitted to your school nurse in the 10 days before your school's vaccination clinic. Students can also bring the form with them on the day of their school’s clinic.
