Clinic at Kanapaha
Hi! My name is Lee Shimansky, and I am very excited to be the new School Nurse here at Kanapaha.
The clinic at Kanapaha is located in the front office. The clinic is open every day from 8:30 am to 3:30 pm. Our main function as the school nurse is to promote, protect and support the health of students and staff. We also provide health screenings for the 6th grade and others as needed, which include: Vision, Hearing, Scoliosis, Height and Weight, review medical records and review and update school required immunizations. if your student requires medication at school, a parental request for Medication Administration permit is to be completed by the Parents for all medication therapy. Forms are available here and in the clinic..
It is important to have an emergency card on file for your student. These are updated annually and go home at the beginning of the year. Current phone numbers are very important so that we may contact you in an emergency. I look forward to supporting the health of your student.
